What is an 'action plan' and when does the assessor  write one?

If a student is not doing well during an assessed placement an 'action plan' should be drawn up by the mentor and supporting staff in agreement with the student. The action plan will identify the learning deficit/s the student has and ways in which those deficits can be addressed.

It should be recorded within the students assessment document and contain objectives and dates by which they should be achieved. The mentor and student should sign to say the plan has been agreed, when it is devised, and each element can be signed once it is achieved.

The action plan should reflect the criteria and objectives set out in the students practice assessment documentation.

What are the students responsibilities in terms of practice assessment?

Students must make the practice assessment documentation available to you.

Most courses require that they must self assess before the mentor assess them. 

It is the students responsibility to ensure that the assessment process is completed within the time frame given to them by the University (the submission date) and they should negotiate dates with mentors to ensure that this is done in a timely fashion.

What do I do if a student goes off sick?

Keep a record in your practice area of students attendance and any sick leave/absence that occurs. It is the students responsibility to inform the relevant University staff that they are off sick from practice.

If you are concerned about a students sick leave discuss this with them and if necessary contact the link lecturer or practice educator from the University. You can also contact the students personal lecturer if necessary.

Students are expected to be punctual, reliable and trustworthy and any concerns on these qualities should be discussed with them and reflected in the practice assessment documentation.
