Initial screening & Annual Review

This declaration is to be completed by all students at the start of the course and again each year during their studies.

You should read the standards of professional behaviour and conduct which are expected of you by your professional body and to seek clarification from your Personal Academic Tutor if you are unclear. Information can be found within the Course Handbooks, the University Student Code of Conduct (Disciplinary Policy 2021), and The Code: Professional standards of practice and behaviour for nurses, midwives and nursing associates (NMC 2018) or Standards of conduct, performance and ethics (HCPC 2016). Declaration

I the undersigned student declare that:

• I am/or remain of good character
• I have completed a DBS form, and that I understand my remaining on the course is subject to this check being clear or passed by the School’s DBS panel
• I have reported to my Personal Academic Tutor any incidents or events that may result in an adverse DBS clearance, such as:
• I have accepted a caution from the police or have been charged with, or found guilty of, a criminal offence;
• another organisation responsible for regulating a health or social-care profession has taken action or made a finding against me; or
• I have had any restriction placed on your practice, or been suspended or dismissed by an employer, because of concerns about my conduct or competence.
• I am/remain in good health
• I have/will attend an occupational health assessment as required
• I can confirm that I understand the need to inform my Personal Academic Tutor of any change to my health status that may affect my performance or continuance on the course and that I agree to an occupational health referral, should the need arise
• I undertake to abide by any recommendations made by the occupational health consultation

I the undersigned student declare that I have disclosed to the accompanying University representative any issues that I feel may impact upon my ability to complete the course, and that I have sought appropriate advice and support as directed.
